Pulmonary hypertension is a severe illness associated with increased pulmonary vascular resistance and pulmonary artery pressure, which may cause heart failure and death.[1-3] Inhaled nitric oxide or a continuous i.v. infusion of epo-prostenol is commonly used to treat pulmonary hypertension, but both options are expensive and difficult to use.
Several case repo:: Manqol.com ::rts and small clinical studies have demonstrated the effectiveness of sildenafil in infants and children with pulmonary hypertension.[4-10] Sildenafil is a selective inhibitor of cyclic guanosine monophosphate-specific phospho-diesterase-5, which is present in the pulmonary vasculature, thereby enhancing nitric oxide-mediated vasodilation and decreasing pulmonary vascular resistance.[11,12] A recent randomized, double-blind, placebo-controlled, crossover study in adults with primary pulmonary hypertension found that patients taking sildenafil had substantial improvements in cardiac index, exercise tolerance, and quality of life (dyspnea and fatigue) versus the placebo group.[13]
Sildenafil citrate is currently available as 25-, 50-, and 100-mg tablets; no suspension is available for oral administration. A liquid dosage form would allow for the administration of individual doses, based on body weight, to infants and young children, who are unable to swallow tablets. In the absence of a commercially available liquid preparation, the tablet must be ground into a powder, which should be mixed in an appropriate vehicle just before administration to patients by nurses or caregivers. This approach is tedious and time-consuming and may be associated with the administration of incorrect doses.
